He knows why now. His heart was working at 60 per cent of its full capacity. He learnt of that in the summer, just as the tragedy of his father Jaap leaving this life, through cancer, was really hitting home. Struve is a giant, a deeply committed fighter, an enthralling and intriguing human being to be around, and let it be known, a son who truly loved his father.

Good news is on the way, though. His recovery is almost complete. In London this week, we spent the best part of two days together. The 7ft giant selects his food meticulously, insists he would probably be a Vegan if he didn't need meat for the protein, and is thoroughly enlightening to be around.
At times, you find yourself looking at a being who seems to have come from another planet, and may well be a long way ahead of his time.
He's a meticulous researcher. The passing of his father, and the illness he fought, has made the Dutchman strive to understand the cell system in the body.

At one point, he was explaining how he believes he could have done more for his father with the things he had studied than the chemotherapy his father underwent.
Struve is not vindictively anti-corporate, but has an acute sense of seeing people scrabbling relentlessly for money. To put in context, what he went on to say was that he feels extremely privileged to be doing a job he truly loves, self-created, his own boss, self-motivated, and testing himself to the core.

Great human being. Yet totally committed fighter. A brutal talker when it comes to fighting. It is a strange juxtaposition, just as it is when you move the seat back in your car to its limit, use the remote to lower the back of the seat to 60 degrees and his knees are still there, as high as the dashboard. He's damn big. But totally comfortable with it.

His size enabled him to get going early in MMA. Aged 15, sparring with men in Bob Schreiber's gym. And boy, does he have some stories about Bob, who, back in the day, seemingly went anywhere in the world to fight anyone, with gloves or bare knuckles, with rules, and in fights where rules were barely conceived. Stefan went with him to some of those.

So, a year has passed. He is almost mended. His huge heart back and working at over 90 per cent capacity now. "It's crazy to think it's been a year since Japan... so much has happened. Time flew by. It didn't go as planned, of course, in Japan. Obviously he broke my jaw in the third round and we just couldn't figure out why I was so tired."
"I got sick before the fight, but I should not have been that tired. I should not have been that tired at the end of the first round even being sick. So I was trying to figure that out but we really couldn't figure it out, except for blaming the illness."
"Nine days after we got back in Holland I got surgery on my jaw. They finally put it back together; they put titanium in there and it healed up and I was back on the road to recovery hoping to fight in about six months."
But he still didn't feel right. "I was just feeling really low on energy, and feeling lethargic, not my usual self. I went to the hospital and the first check was good but the doctor wanted me to do a sports test and an echo of my heart which they didn't do before that time."
"So I went back the next week and they find that I have a bicuspid aortic valve. I think you are born with it and it's crazy to find that after such a long period of time, especially when you look at all the stuff I have done in the last 7 or 8 years as a pro fighter. So they found this and they found out my heart is functioning at about 60 to 70 %, and there is about 30 to 40% leakage back into the heart."
It was terrible news. "The first thing the doctor said was that my career was going to be over and I was going to go into surgery about two or three weeks later because the leakage back into the heart was so severe."
"They kept me in the hospital for a week and then they let me go with medication because they had monitored me for a week and they saw that I didn't develop any heart failure because of the leakage and also because the size of my heart being bigger then it should be."
The medication paid off. "I go back a couple of months later after being on the medication and everything turns out right then. The doctors were really surprised at how I was doing and the leakage was reduced greatly."
"The doctor told me that there was a chance that I could possibly be fighting again so that was awesome. But around that time things went downhill with my dad who had been battling cancer for almost a year."
"I was really happy I could tell him the good news because it was one of the last really good things I could really please him with. A couple of weeks later he passed away because of the cancer so it was a really tough time."
"I should have been focused on my own health but really couldn't because my dad was so sick."

Terrible times. But on January 2, this year, the best news possible. Struve was told that the function of his heart which has been an issue was now at over 90 per cent. Struve will go to the States in the coming days and look to get the all-clear from the UFC.
"Hopefully I'll get cleared to fight and to be back in early spring I think that's about it for my year. Pretty active."

Thirteen times he has fought in the UFC. Nine wins, four losses. He accepts the loss to Junior Dos Santos. It was his UFC debut. Some context now for his debut as a 21 year old. But he believes the Hunt, Roy Nelson and Travis Browne fights should have been his.
"I can't wait to come back. It's not only a physical change and improvement, there's a mental improvement in my opinion. I'm really going to come back the best I've ever been. I really feel that I'm one of the best heavyweights in the world and I cannot wait to prove that, and work my way back up there and fight my way up to the title shot."

So, to a little of his philosophy...on nutrition, on money, on living one's life. "I just think that a lot of diseases and illnesses that you see nowadays are because of the way people live and the way people eat. They eat the wrong things at the wrong times and they just live an unhealthy life."
"I think so many people are too busy working their arse off, or making money, they just live their life in a system that was created for them they don't really live the life they want to live."
"Everything seems to be about money, you know. Of course I need money to live life the way I want to live because I'm living in a society where you need to have money. Everybody is waking up to go to their job every single day because they have to eat... you cannot do it without it because you end up in the shit. It's as simple as that. But I would have no trouble living in a house with a nice big garden with my own vegetables and fruits, live by the sea catch my own fish. That would be amazing."
He'll get there. He was 7ft tall a year ago. But he seems a little bigger now, and quite clearly, a much more enlightened gentle giant outside the Octagon. But he promises that inside it, he is ready to go to war again.
