Edit: It's meant to be ISAAC talking! He starts by saying "Hello Esau, this is your (backwards speech)" That backwards speech as someone pointed out is likely "father". In the bible Esau is the son of Isaac.
So the narrator has the same voice? That explains the pill/card narrator! It's not a random disembodied voice, it's ISAAC talking to himself.
Edit 2: Another thing to consider, Isaac's half brother Ishmael/mother were expelled after mocking Isaac. He supposedly wandered lost for a while. Perhaps the question could be "Where is Ishmael"
Edit 3: As /u/CNuggs pointed out, there is an ending that shows Isaac growing up and his son starts the cycle again. Could his son be named Esau? What does this mean?
Edit 4: An important interaction between Esau and Jacob (his brother) occurs in Genesis 25:19-34. Basically Esau gives up his birthright to Jacob. He asks "What good is my birthright to me?" Maybe this a good question to ask?
