Code violations were alleged in eight of the 12 fires under examination, CalFire said.
A CalFire spokesman, Michael Mohler, said agency officials looked for specific lapses ranging from maintenance and equipment failures to improper vegetation clearance, but he declined to discuss specific findings because the matter remains under investigation.
Last month, the agency similarly issued findings for the first four fires it examined from the North Bay incident, concluding those, too, were triggered by trees falling into PG&E power lines. Alleged clearance violations were cited in three of those instances. Mohler said investigations were still under way for several remaining fires from last October’s outbreak.
PG&E said it meets or exceeds regulatory requirements for maintaining its utility poles, inspecting every electric transmission and distribution line at least once a year and pruning or removing some 1.4 million trees annually.

Authorities released a list of 297 people still unaccounted for. "We'll be here for several years working this disaster," said the administrator of FEMA.
Authorities made public a list of 297 people still unaccounted for Wednesday night as they announced that the number of people who had been killed in the deadliest wildfire in California history had grown to 56.
Butte County sheriff's officials earlier in the day published a list of 101 missing people. At a news conference early Wednesday night, Sheriff Kory Honea said 29 further names would be added later Wednesday night, for a total of 130. But when the updated list (PDF) was published late Wednesday evening, it bore 297 names.
The sheriff's office said no one was available to discuss why so many more people were listed as missing in the intervening three hours.
Meanwhile, eight more sets of human remains were found Wednesday, the sheriff's office said, and 7,600 homes have been destroyed since the Camp Fire ignited last Thursday morning. The fire had consumed 135,000 acres by Wednesday night and remained only 35 percent contained.

The Trump administration says that water in California should be prioritized for wildfire response instead of endangered species protections, even though state officials say they have enough water to fight the fires.
The Department of Commerce announced a directive on Wednesday that says the National Oceanic and Atmospheric Administration is taking over management of water in California in areas affected by the ongoing fires.
The change announced Wednesday would allow federal agencies to expedite decisions about water under an emergency provision of the Endangered Species Act.
The directive specifically says that the National Oceanic and Atmospheric Administration, which includes the National Marine Fisheries Service, will take over management of water during the wildfire emergency in California. Commerce Secretary Wilbur Ross directed the Service to tell other federal agencies "the protection of life and property takes precedence over any current agreements regarding the use of water in the areas of California affected by wildfires."
"Today I directed the National Marine Fisheries Service to facilitate access to the water needed to fight the ongoing wildfires affecting the State of California. One of the fires, the Mendocino Complex Fire, has developed into the largest in the state’s history, consuming nearly 300,000 acres in Northern California. American lives and property are at stake and swift action is needed," Ross said in a statement.
But there is concern that the Trump administration is actually wading into a political conflict about water in California between the salmon fishing industry and farmers. The two groups have been going back and forth for years on whether water use should be limited to protect salmon populations crucial to fisheries or should be diverted for agricultural use.
"Secretary Ross’s directive is nothing more than a smokescreen designed to weaken these protections that NMFS’s scientists determined are necessary to keep these native fish from going extinct. It’s almost like the extinction of these creatures is their real goal, so that they no longer have to leave any water in rivers, but can divert it all to corporate agribusiness. The people of California won’t stand for Trump destroying our precious resources to line the pockets of his corporate buddies,” senior director of the water division for the Natural Resources Defense Council Kate Poole said in a statement.
The National Marine Fisheries Service works with California Fish and Wildlife to protect the delta smelt, a small fish that is seen as an indicator of the health of fish in rivers in central California, and Chinook salmon. The agencies work to reduce the amount of water that is removed from the rivers to protect the fish population.
Secretary of Commerce Wilbur Ross issued a directive on Wednesday telling the National Marine Fisheries Service and other federal agencies to prioritize "the protection of life and property" over endangered species in the areas of California affected by wildfires, saying that public safety takes precedence.
